Passport Services
Members of our Passport Services team are authorized by the U.S. Department of State to process the DS-11 application.
All visitors to the Altadena Libraries must follow safety protocol to prevent the spread of COVID-19. Please review the required protocol at www.altadenalibrary.org/reopening.
PLEASE NOTE: We can only process the DS-11 application. We cannot process DS-82 Renewal applications. Applicant will need to mail the renewal application to the processing center. This form is to be used for adult renewals if you were over the age of 16 when the passport was issued and your passport expired less than 5 years ago.
Passport Services Hours
Please call (626) 798-2950 to schedule a passport appointment. Passport services are available by APPOINTMENT ONLY.
Monday – Saturday: 9:00am – 4:00pm
Office hours may vary without notice – It is required to have a confirmed appointment before arrival.
Appointment Guidelines
- Face masks are required at all times, and social distancing (6 feet) must be observed.
- The Passport Office will be cleaned and sanitized after every appointment.
Application Guidelines
To prepare for your appointment, you must bring your application already filled out and have payment ready in exact amounts (credit card, checks, money orders; cash is not accepted at this time).
Bring the following with you to apply for a passport:
- Hospital birth certificates, California birth abstracts, and Texas birth abstracts are not accepted.
- Social Security number is required on all applications, and all applicants must be present.
- Proof of U.S. citizenship (original/certified birth certificate, or your old passport, or your naturalization certificate), Please note- Photocopies are not acceptable.
- Proof of identify (valid driver’s license or state I.D.)
- One passport color photograph (2 in x 2 in)
- One Payment to U.S. Dept. of State (check or money order only)
- One Payment to the Altadena Library District (check, money order, credit or debit)
- Complete DS-11 Form – Section 1-21 only. Pick up the form at the Library Passport Services office or download the form. All applications MUST BE completed prior to your passport appointment, including all Social Security Numbers and Parents Birth Date and Birth Place.
- Children under 16 – both parents must be present with the child. Submit a notarized statement of consent for those unable to appear. (DS-3053) along with a photocopy of the front and back of the valid identification presented to the notary.
- An original birth certificate is needed for new or renewed passports. All applicants must be present.

Processing Times
Routine: 4-6 weeks*
Expedited: 2-3 weeks and an extra $60*
*Consider the total time it will take to get a passport when you are booking travel. Processing times only include the time your application is at a passport agency or center.
- It may take up to 2 weeks for applications to arrive at a passport agency or center. It may take up to 2 weeks for you to receive a completed passport after it is printed.

NEW LOCATION! Where is the Passport Services Office?
The Passport Services office is located in the Altadena Main Library (600 E. Mariposa Street). The entrance opens up to the northern section of the parking lot near the loading zone.
If your appointment is before the Passport Services office opens at 9:00am, please wait outside and a member of our Passport Services team will open the door for you at the start of your appointment.
